Job Description:
A company is looking for a Field Case Manager (Nurse or Social
Worker).
Key Responsibilities
Manage and coordinate care for patients in a remote setting
Conduct assessments and develop individualized care plans
Collaborate with healthcare teams to ensure effective discharge
planning
Required Qualifications, Training, and Education
Bachelor's degree in behavioral health or human services
required
Minimum of 2 years of experience in behavioral health or social
services
Case management and discharge planning experience preferred
Managed care experience preferred
Proficiency with standard corporate software applications
