Job Description Livingston Memorial is looking for a Full Time,
40 hours a week, Director of Patient Care Services to support our
Hospice Team! We are proud to announce that Livingston Memorial’s

County residents! Summary This position directs all aspects of
LMVNA’s Hospice patient care services, which includes but is not
limited to nursing, social work, home health aide, and specialty
nursing. This position interacts with all Livingston departments,
external community entities, and regulatory agencies to ensure
compliance with licensing accreditation requirements, Title 22,
Medicare Conditions of Participation, professional licensing and
certification standards of care, and maintains LMVNA’s policies and
procedures in accordance with regulations and quality standards of
patient care and support processes. This position assures that all
hospice care for patients reimbursed by Medicare, Medi-Cal, and
private insurance sources is provided in accordance with all
compliance and regulatory requirements. Hospice implements and
manages the Hospice benefit and acts as payor of services as
appropriate. What You'll Do: Interprets LMVNA’s policies and
programs to clinical services personnel, members of the community,
and fiscal intermediaries. Provides direction and management within
the department and delegates authority as appropriate. Works with
QI Department on the development and implementation of standards
and quality improvement mechanisms that assure safe and quality
service to patients and their families. Participates in studies,
analyses, and research projects relating to LMVNA’s services as
applicable. Conducts appropriate evaluations of Hospice staff per
policies. Participates in the process to achieve and maintain
internal and external customer satisfaction. Uses data and
statistics to analyze effectiveness of Hospice programs and
services. Interprets operational indicators to detect census
changes and increases or decreases in volume that could impact
staffing levels, revenues, or expenses. Demonstrates sound,
effective judgement problem solving issues and ownership for their
resolution. Completes field evaluations for other nursing
directors. Maintains knowledge of Federal and State regulatory
changes related to Hospice and Palliative services and implements
necessary changes in policies and/or processes to maintain
compliance. Assists in the development of departmental and
organizational goals. Works closely with Director of Care
Transitions to manage and promote hospice with the community to
further LMVNA goals. Ensures adequate qualified staff to meet
patient care needs and actively collaborates with Human Resources
in planning, recruiting, and hiring efforts. Ensures that customer
needs are identified and met productively and positively. Manages
departmental operations within budgetary constraints. Effectively
uses interpersonal skills for building and developing positive
relationships. Participates in the planning of the annual budget
process and development of the Strategic Plan for patient care
service departments. Able to express ideas well in oral as well as
written communication. Maintains a positive, professional, and
respectful attitude. Performs other related duties as assigned.
Principal Accountabilities : Ensures compliance with state,
federal, and accreditation regulations regarding hospice care.
Assumes responsibility for the direction and accountability of all
aspects of patient care services within the following programs:
clinical professional and paraprofessional nursing services, social
work services, and specialty professional nursing services. Assists
the President/CEO, COO, and the Compliance Department with meetings
and developing the agenda and necessary documentation. Collaborates
with Compliance/Quality Improvement Department to obtain quality
based outcomes in all areas of clinical performance. What We're
Looking For Valid California Registered Nurse license. Bachelors of
Science in Nursing or other related health field Minimum three (3)
years current experience in a home health/hospice agency, primary
care clinic, or health facility such as a hospital Minimum one (1)
year of which was in a supervisory or administrative capacity.
Valid Basic Life Support certification. Certified Hospice and
Palliative Nurse (CPHN)preferred Current and demonstrated knowledge
of Medicare, accreditation, and Title 22 regulations, and available
standards of practice Good verbal and written communication skills.
Valid Driver License and auto insurance High level of attention to
details. Able to work independently, take ownership and
responsibility of tasks. Strong professional ethics, discretion,
