Our Mobile Primary Care team delivers care directly into homes, assisted living, and skilled nursing facilities — serving complex senior populations through a value-based, panel-driven model. What You'll Do • Deliver Mobile Primary Care to seniors in homes and senior living settings • Manage complex chronic disease in Medicare populations • Drive patient engagement, retention, and outcomes • Collaborate across interdisciplinary teams • Strengthen facility and community relationships What We're Looking For • Active, unrestricted Physician Assistant (PA-C) or Nurse Practitioner (NP) license • 2 years clinical experience preferred (geriatrics or primary care) • Strong preference for home/SNF/ALF experience • Demonstrated ability to build and manage a patient panel • Leadership potential or prior mentoring experience • Entrepreneurial mindset with strong clinical judgment Compensation & Growth • Guaranteed Base Salary Productivity • Full benefits package • Clear advancement into clinical leadership If you're ready to build something — not just work shifts — we'd love to connect.
