Job Description:
For more than 55 years, Family Health Centers of San Diego’s
(FHCSD) mission has been to provide caring, affordable,
high-quality health care and supportive services to everyone, with
a special commitment to uninsured, low-income and medically
underserved persons. FHCSD is one of the top 10 largest federally
qualified health centers (FQHCs) in the country. We operate more
than 90 sites across San Diego County, including 29 primary care
clinics, 23 behavioral health facilities, 10 physical
rehabilitation clinics, nine dental clinics, five vision clinics,
four outpatient substance use treatment programs, three mobile
medical units, two mobile counseling centers, two urgent care
centers, and a pharmacy. Our staff provides care to over 227,000
patients each year, of whom 91% are low-income and 29% are
uninsured. FHCSD provides care to all. Services include, but are
not limited to adult care, chronic disease management, pediatrics,
comprehensive women’s care including obstetrics, dental, vision,
case management, physical rehabilitation, speech therapy for
children, vaccinations, infectious diseases, behavioral health,
substance use counseling and a host of specialty services including
cardiology, podiatry, endocrinology, dermatology, among others.
FHCSD also offers supportive services to those who are unsheltered
and in need of intensive case management. The breadth of our clinic
locations, services and programs has grown over the last five
decades, making us the largest community clinic provider of health
care to the uninsured in the county and one of the top 10 largest
community clinic organizations in the nation. We are also the
largest health care safety-net provider, largest school-based
health care provider and the largest mental health provider in the
San Diego region. Responsibilities: Responsible for comprehensively
addressing the needs of clients with the most complex challenges
affecting their health related to homelessness, unstable and unsafe
housing, food insecurity, and/or other social needs. Develop a
comprehensive, individualized, person-centered housing support
plan, based on an individualized assessment ensuring the plan is
reviewed, maintained, and updated under appropriate clinical
oversight. Determine appropriate services to meet the needs of
patients to include addressing social determinants of health needs
effecting housing. Utilize best practices with patients
experiencing homelessness that have complex health, disability,
and/or behavioral health conditions that may include Housing First,
Harm Reduction, Progressive Engagement, Motivational Interviewing,
and Trauma-Informed Care. Assist patients with obtaining or
searching for housing that covers completing housing applications,
securing required documentation, assist with benefits advocacy and
presenting options. May assist clients in supporting SSI
application process in aligning eligibility, eligibility
guidelines, obtaining identification and documentation for
Supplemental Security Income eligibility, and overall support with
the SSI application process. May assist with identifying and
securing available resources for subsidizing rent, identifying and
securing resources to cover expenses, or assisting with requests
for reasonable accommodation. Assist with identifying,
coordinating, securing, or funding one-time services and
modifications necessary to enable a person to establish a basic
household that expands beyond room and board. Provide tenancy and
sustaining services to assist the client in maintaining safe and
stable tenancy once housing is secured Provide early identification
and intervention for behaviors that may jeopardize housing,
education and training on the roles, rights, and responsibilities
of the tenant and landlord, coaching on developing and maintaining
key relationships with landlords/property managers with a goal of
fostering successful tenancy, or coordination with the landlord and
case management Provider to address identified issues that could
impact housing stability. Collaborate with multidisciplinary care
team in coordinating a wide array of outreach, health care, case
management, patient engagement functions in supporting and
connecting patients of the Homeless Population with internal and
external resources. Resolve any barriers related to necessary
resources in supporting this population. Interact with internal and
external stakeholders in developing and sustaining relationships to
service patients’ needs and resources. Perform other duties as
assigned. Requirements: Bachelor’s degree in social work, public
health, science, or related degree required. 2 years of experience
with health education or working in the medical/healthcare related
field. Or equivalent combination of education and experience that
provides the skills, knowledge and ability to perform the essential
job duties. Traveling between sites and other locations is required
as an essential function of the job. Must have a car, a valid
California driver’s license, and proof of minimum levels of car
insurance as required under California law, although limits of
$100,000 are recommended. An acceptable driving record is also
required. California law requires all drivers to obtain a valid
California driver’s license within ten days of establishing
residency. Mileage and other reimbursement governed by policy.
Knowledge of adult health coaching and outreach techniques, best
practices and tools. Ability to translate best practices into
meaningful, culturally appropriate resources. Ability to engage
clients in meeting their healthcare needs to include cultivating
patent connectedness with FHCSD services through care coordination
and case management strategies. Ability to exercise discretion and
maintain confidential information. Ability to work in a fast-paced
environment in seeking solutions and service connections for
clients. Ability to display cultural sensitivity and respect to
ethnically and culturally diverse populations. Knowledge of a wide
array of services and resources related to outreach/patient
engagement, healthcare, case management, and/or care coordination.
Knowledge of the community resources, health and social services
systems in San Diego County, and possess skill to establish working
relationships with community partners. Possess critical thinking
skills in finding solutions to problems/addressing barriers. Strong
interpersonal and customer service skills with the ability to
interact empathetically with persons who possess diverse levels of
socio-economic and education backgrounds. Strong verbal and written
communication skills to include the ability to communicate
effectively with those who speak English as a second language.
Bilingual in English/Spanish preferred. Basic computer literacy to
